The Issue

Unacceptable Neglect of Excruciating Pain Experiences:

The current state of pain management during IUD insertion in Ontario fails to recognize and acknowledge the pain experiences of women. While few women may have a relatively painless insertion, many others undergo agonizing pain, which can be traumatizingly painful. Disregarding the severe pain experiences of women perpetuates a system that neglects their well-being and hinders access to effective pain management.

On a pain rating scale of 0-10, at least 50% of the 100's of respondents who have shared their insertion experience with me rated their pain as a 7 or higher with the associated descriptions of:

  • Extremely painful
  • Almost unbearable
  • Excruciating
  • Several people have reported the pain as worse than childbirth or broken bones.

I would like to make Ontario's current regulations require the following:

  • More detailed explanation of what the insertion and removal of an IUD entails. Without fully explaining the real potential pain, how can consent truly be given?
  • More pain relief options offered for IUD insertion and removal at various birth control and gynocological centers as standard practice.
  • Not everyone will choose to have additional pain relief, but it should be an option, especially for patients who ask about it instead of being dismissed.
    • Without all the risks and prcedures explained prior to insertion, INCLUDING RISKS OF PAIN consent for insertion is not given for this procedure.

Media outlets, such as Chatelaine, have shed light on inadequate pain management during IUD insertion, highlighting the need for improvement. Clinics in the USA, Halifax, Vancouver, and Montreal offer options for pain management. These procedures include freezing or scripted medication before the procedure. Unfortunately, there are no clinics in Ontario that offer any pain management options or even acknowledge the intense pain many women experience. Women in Ontario are instructed to take two Tylenol or ibuprofen before insertion, which they claim will feel like period cramping. Studies and testimonies have proven that Tylenol and Ibuprofen do not work for about 50% of women, who end up experiencing excruciating pain. Furthermore, numerous women have taken to social media platforms like TikTok, sharing their traumatizing experiences during IUD insertion. These testimonials, such as this one, provide firsthand accounts of the pain and suffering endured by women, further emphasizing the urgency to address pain management practices during the procedure.

Women Are Not Warned About Pain Risks:

It is essential that healthcare providers in Ontario acknowledge the diverse pain experiences during IUD insertion and take immediate steps to provide comprehensive pain management options. Pain risks and the full procedure should be explained to patients considering an IUD. Also, management protocols should be tailored to address the different needs of women, considering factors such as previous childbirth, sensitivity, and individual pain thresholds. Local anesthesia, conscious sedation, or other proven techniques must be readily available to ensure a more comfortable and compassionate experience for all women.

Research from the National Center for Biotechnology Information (NCBI) underscores the importance of pain management strategies during IUD insertion. The study highlights various techniques, such as the use of local anesthesia and pharmacological approaches, to alleviate pain and improve the overall experience for women. Implementing evidence-based pain management strategies based on research findings can significantly enhance the care provided during IUD insertion.

Ontario Continues to Sell This Painful Procedure, Ignoring the Pain Experienced by Women

Ontario's neglect of the pain experiences during IUD insertion is unacceptable. It is crucial that pain management practices are improved to address the diverse needs of women. By acknowledging and accommodating these varied experiences, we can ensure that all women, regardless of their reproductive history, receive the compassionate care they deserve.
