The Issue
Joshua, a vibrant 16-year-old, walked into the emergency room with a heart rate of 256 bpm. His final words to his parents were, "I don't want to die." Joshua was given a sedative and 11 minutes passed for the first attempt of cardiac medicine was given. This was unnecessary medication for a child that was only scared when the doctor said he would die without the slowing of his heart. Unfortunately, due to medicine being delayed cardioversion was postponed for an agonizing 27 minutes. During this delay, Joshua went into cardiac arrest. Though he was temporarily revived, the irreversible damage had been done. Joshua tragically passed away 15 days later.
The devastating loss of Joshua highlights a critical flaw in our healthcare system: the absence of mandated protocols to ensure immediate and appropriate emergency care for pediatric patients experiencing supraventricular tachycardia (SVT) exceeding 220 bpm. This is an urgent call for "Joshua’s Law"—a set of necessary legislative measures to not only prevent future tragedies but also uphold the integrity and accountability of our medical practices.
The first measure of Joshua's Law seeks to mandate immediate cardioversion or equivalent lifesaving techniques for children with severe SVT, thereby reducing the likelihood of cardiac arrest and increasing chances of survival. Time is of the essence, and every second counts in these critical situations. Delays, often caused by unnecessary sedation procedures or miscommunication, must be eliminated.
