Implement sepsis 6 bundle training at UHL Hospital

The Issue

 

“Establish an Independent Investigation into Sepsis Failures at University Hospital Limerick”

University Hospital Limerick (UHL) has seen repeated, preventable deaths linked to delayed or inadequate sepsis recognition and treatment.

Families across the Midwest have suffered unimaginable loss because basic, internationally recognised sepsis protocols were not followed.

Recent cases — including young people whose lives should have been saved — show the same pattern:

Failure to recognise sepsis early
Failure to apply the Sepsis Six Bundle within 1 hour
Failure to escalate care to senior clinicians
Delays in antibiotics, fluids, monitoring and critical investigations.


HOW ARE MEDICS NOT DOING THIS, IT SHOULD BE SECOND NATURE TO THEM.


These systemic issues have continued despite repeated warnings since 2009 from clinicians, campaigners and families.

We, the undersigned, call on the Minister for Health and the HSE to immediately:

Launch an independent investigation into sepsis management at UHL, including case reviews of all sepsis-related deaths in the last 10 years.
Mandate Sepsis Bundle training for all frontline staff at UHL, with public reporting of compliance rates.
Audit sepsis care pathways to ensure patients receive antibiotics, fluids, blood cultures and escalation within the first hour.
Publish all findings in full so families receive transparency and the public can have confidence in UHL’s safety.
No family should lose a loved one because proven, life-saving sepsis care was delayed or forgotten.

The people of the Midwest deserve better — and they deserve answers.

We demand urgent action now.
