USMLE/NBME/ECFMG: Promote Equality By Stop Reporting the USMLE STEP 2 CS Attempts.
The Issue
Please stop reporting the USMLE step 2 CS attempts.
- On March 16th, 2020, USMLE suspended the STEP 2 CS exam and subsequently discontinued it on January 26th, 2021 leaving individuals who have failed the exam pre-pandemic stranded and without the ability to remediate to show improvement.
- Currently, medical students/graduates who are American and International no longer have to take the STEP 2 CS in order to obtain medical licensing. However, STEP 2 CS failures for candidates who failed the exam pre-pandemic are still being reported on the USMLE transcript, which hinders their progress through their medical education.
- Additionally, the attempts of the English proficiency exam (The Occupational English Test or the OET) that International Medical Graduates now take to get the ECFMG certificate are NOT reported. Thus candidates can now have multiple attempts on OET, but their attempts won't be shown on their residency applications.
- THOUSANDS of American Medical Graduates as well as US & Non-US International Medical Graduates remain unmatched each year, and reporting the USMLE STEP 2 CS attempts will further hinder their chances adding to massive student debt and doctor shortage. Adding STEP 2 CS filters during residency applications is another way these individuals are further marginalized.
- THIS IS UNFAIR TOWARDS THOSE WHO HAVE FAILED CS IN THE PAST AND HAVE TO FACE CS ATTEMPT FILTERS.
- The most plausible solution is to expunge CS attempts from the USMLE transcript.

ERAS and NRMP should also consider removing the Step 2 CS attempt filters that are provided to programs.
