Conduct a thorough inquest into the case of Chazz Petrella to identify service delivery gaps in the Ontario Child & Youth mental healthcare system which resulted in a preventable death (by suicide) despite Chazz being registered with nine agencies.

The Issue

The Petrella family in Cobourg, ON were offered unacceptable solutions – abandoning custody of their mentally ill child to the Crown’s care when that care is severely limited to very stringent basic needs.  The laws that govern this area must be revised to reflect current needs in the province so that timely, acceptable help is readily accessed by those who need it. Legislation that is outdated must be revised to reflect current needs that offer acceptable, workable solutions to those in dire crisis. 

I strongly urge the Ontario Minister of Child & Youth Services to get the Petrella Family Inquest underway as soon as possible to pinpoint the gaps in service delivery so they are filled to prevent further tragedies like this from happening to countless unreported others.
