The Issue

Tennessee's lethal injection process broke down on September 30. Staff spent about an hour placing IV lines in Christa Pike and gave her two doses of pentobarbital, and she survived. She is in critical condition at an undisclosed hospital. The Tennessee Department of Correction (TDOC) says it followed its protocol. If that is true, the protocol itself is the problem.

The warning signs were there in advance. Pike's legal team says medical experts predicted that her blood condition and hard-to-reach veins could cause the IV to slip or push through the vein, sending the drugs into her arm instead of her bloodstream. Her attorneys believe that is likely what happened. They also raised concerns about degraded drugs and the lack of emergency medical care in the room, all under rules the public is not allowed to see.

Governor Bill Lee said Tennesseans expect executions to be carried out in a way that is "not only legal and constitutional, but is effective," according to USA TODAY. On September 30, the state met none of those standards. Lee has paused executions for the rest of 2026 and ordered an outside investigation, but has not said who will lead it.

Whatever anyone believes about the death penalty, a sentence carried out in the public's name should never become an hour of failed needle sticks. Governor Lee, TDOC and the Tennessee General Assembly should release the full findings of the investigation and replace the current protocol with a more reliable method reviewed by independent medical experts. They should also screen every prisoner's medical risks in advance and make drug testing and procedures public.
