Revise the EACTS/ESC left main coronary artery guidelines
The Issue

Dear Professors Licht and Pagano,
We are writing to share our concerns with regards to the latest events surrounding the EXCEL trial analysis and its implication for clinical practice guidelines and for the treatment of patients with left main coronary artery disease.
On July 16th 2020, the NEJM published a letter in which the EXCEL trial leadership present the incidence of myocardial infarction defined by the 3rd Universal Definition at 5-year follow-up. The new data show a significantly higher risk for this complication in the PCI group. This is in clear contrast with the original EXCEL publication that used the protocol definition of myocardial infarction and reported a similar incidence in the two arms.
This finding raises several questions:
- In the EXCEL protocol the analysis of myocardial infarction using the 3rd Universal Definition was a pre-specified secondary end point. Why was this not analyzed and described at the time of the original manuscript?
- Why did the authors repeatedly claim that re-adjudication of the myocardial infarction data using the Universal Definition was impossible?
- Were the ESC-EACTS Myocardial Revascularization Guidelines adversely influenced by this important data omission?
- Have there been any adverse consequences to patient care as a result?
We request that the ESC-EACTS Myocardial Revascularization Guidelines be reviewed as a matter of real urgency. In addition, we would ask that you work with the major surgical and cardiological organizations collaboratively to provide accountability for clinical trials conduct and interpretation. This is extremely important to safeguard our patients.
Finally, with regards to the EXCEL trial, it is essential that the data be made publicly available for truly independent scrutiny and the results of the original reports should be amended with the addition of the Universal Definition data.
We trust that EACTS leadership will be sympathetic to this “Call for Action”.
