Petition to Support Continued Access to Care for Elan Sleep LLC (Rebis Health) Patients with United Healthcare
To Whom It May Concern,
Summary of the Situation
Elan Sleep LLC, known as Rebis Health, is a physician-directed sleep medicine specialty clinic that provides evaluation, diagnosis, treatment, and long-term management for patients with sleep disorders. Our practice has provided specialty sleep care services to United Healthcare (UHC) members for over 15 years.
United Healthcare has chosen not to renew its contract with the Boulder Valley Independent Physicians Association (BVIPA), effective March 31, 2026. As a result, Rebis Health will be out of network with UHC as of April 1, 2026, through no action or choice of our practice. We are concerned that our organization is being incorrectly categorized as a testing facility instead of a physician-led specialty practice that provides ongoing clinical oversight and treatment management. This situation is not merely a contract matter; it will directly affect patients' access, continuity of care, and financial responsibility.
Impacted Patients and Scope
This contract change has the potential to affect a significant population across Colorado who rely on established specialty sleep care:
- Rebis Health has served more than 16,000 UnitedHealthcare patients (including UHC, Medicare Advantage, and UMR plans).
- We currently have approximately 1,000 UnitedHealthcare patients scheduled over the next six months.
- Our patients come from more than 400 cities across Colorado, ranging from age 0 to 102.
- Many patients have chronic conditions that require continued specialty care, monitoring, and long-term treatment management.
Without transition protections, patients may face disruption in ongoing care and treatment plans, delays in medically necessary services, difficulty accessing specialty sleep care locally, and higher out-of-pocket expenses.

I support continued access to care patients of Elan Sleep LLC, doing business as Rebis Health, who may be affected by the United Healthcare contract termination.
I respectfully ask United Healthcare to help preserve continuity of care, provide a reasonable transition pathway, and prevent disruption of medically necessary specialty sleep services. I also support recognition of Elan Sleep LLC (Rebis Health) as a physician-led specialty practice that provides ongoing care and essential access to specialty sleep services for patients throughout Colorado.
