Hi, my name is Josephine Scott, I am a 53 year old woman who resides in Rochester NY, suffering with a chronic lung disease.  I have had this incurable disease now for over 15 years.  I have been admitted into the hospital on a numerous of times with this illness.  This last time since I was admitted has been rough for me and took a toll on my life. I made it out of the  hospital alive but now currently staying in a facility on a ventilator to live.  I was told by social workers of the U of R and also hospital staff that I can live with the ventilator in the comfort of my own home up until 3months ago things had changed.  Now I'm being told by these same people that because of the severity of my illness no doctor is willing to follow me home.  Nothing has changed since I've been here, far as my health, so its hard for me to understand as to why things has now changed all of a sudden for a doctor to follow me.  At this present time I do not know where else to turn or who I can speak to on getting me home.  I know I'm not getting any better far as my health but my biggest fear is to expire in a facility.  I want to be in the comfort of my own home with my children and grand children.  I hope that you will understand my story enough to sign the petition and hopes that the right eyes will see this to grant me my wishes.  thank you so much for reading.  signed heart broken.
