The Issue

It's crucial for the competence and reliability of our healthcare systems that we implement mandatory psychological evaluations for all nurses and healthcare providers prior to hire and throughout their years of practice. Introducing periodic evaluations throughout their careers would ensure continuous mental well-being, leading to a safer and more effective care environment.

A psychological evaluation assesses a candidate's psychological fitness for the demanding role, typically involving questionnaires on personality, stress, traumatic events, and symptoms like depression or PTSD, followed by a clinical interview with a psychologist. The goal is to understand emotional stability, stress tolerance, and resilience to ensure a good fit for the job and to provide support before issues become serious. While results are confidential and non-diagnostic for duty purposes, they are crucial for early detection and support.

The psychological evaluation seeks to understand a candidate's:  

Emotional Stability: The ability to remain calm and in control of emotions in stressful situations.  

Decision-Making: The capacity to make sound, quick, and decisive judgments.  

Teamwork and Interpersonal Skills: The willingness to follow a chain of command and work collaboratively with others.  

Integrity and Trustworthiness: Honesty and a commitment to safety and established procedures.  

Flexibility: The ability to think flexibly and adapt to different situations.  

Resilience: The capacity to cope with the inherent stresses of the job. 


Research indicates that mental health disorders affect approximately 20% of the general population, according to the National Institute of Mental Health. These statistics likely extend into the healthcare workforce, where stress and emotional demands are particularly high. By implementing psychological evaluations, we can intervene early, offering support to those in need, while simultaneously ensuring the safety and efficacy of patient care.

A 2023 study published in JAMA Psychiatry analyzed data through 2019 and found that registered nurses (RNs) had a standardized suicide rate of 16.0 per 100,000, higher than the 12.6 per 100,000 found among non-healthcare workers. Recent studies have highlighted the alarming rates of suicide and suicidal ideation among nurses, driven by a combination of high occupational stress, demanding working conditions, and burnout. 


The precedence for such evaluations exists in high-responsibility professions, such as law enforcement, fire departments, and aviation, where psychological screenings are routinely conducted. It's time we bring these standards into the healthcare industry as well, recognizing that care providers carry the weight of patients' lives in their hands.
