NHS Administrative forms are excluding the vast majority of families from being able to complete them.

If a child in your care has to visit an NHS Hospital in England you will be sent an appointment letter and with that letter a consent form to complete.

The categories ask for Mother's Name and Occupation and Father's Name and Occupation. This is archaic and triggering for those families who have same sex parents, a solo parent or whom are raised by other family members due to personal and often tragic circumstances.

I am petitioning for the form to be changed so that it reads Caregiver's Name, Caregiver's Relationship to Child, Caregiver's Occupation and this be written out ONCE as this is all that is needed to convey consent.

This will stop the painful process of having blank spaces for those who cannot fill them. It will stop same sex parents having to misgender one parent in order to take the role of a Father or Mother. It will stop other family members who are Primary Caregivers having to scribble out words and explain themselves time after time after time.

It is a simple step that makes an enormous impact and reflects the diverse nature of a modern society. Only then will the NHS be seen as truly accessible and inclusive - which is something it was always built to be but in recent years has definitely stagnated on.
