
Recent data has revealed that a record one in three GP appointments in England are now conducted remotely. While this approach offers convenience for many, unsurprisingly concerns continue to grow about patient safety, the lack of genuine choice, and unequal access to care.
Jess consulted with her GP practice on 20 occasions in five months. Seventeen of these appointments were conducted remotely. Jess’s Rule highlights the importance of face-to-face examinations and continuity of care.
Jess’s Rule – Three Strikes and We Rethink – focuses on patients who consult with their GP practice for the third time with the same unresolved condition and/or escalating symptoms. At this point, the guidance is that their case should be elevated for review.
Jess’s Rule is calling for:
- A fresh eyes approach – to start again, especially if the initial diagnosis was unsubstantiated.
- A second opinion from a primary care colleague.
- Referral to secondary care where appropriate.
- A face-to-face physical examination if previous consultations have been remote.
- Continuity of care from the same GP wherever possible.
- Listening to the patient and carrying out a complete review of the case.
Our second planned launch date for Jess’s Rule, 18 August, has just been cancelled. We are now looking to September. The launch will happen — we just hope it will be soon, well received by primary care practitioners, and actioned appropriately. It is true that many teams of people, including those within NHS England, the RCGP, and the DHSC, are working hard to make this happen.
