NHS England's new Primary Care Safety Strategy was published today. I was given the opportunity to read and comment upon earlier drafts of the strategy. An account, provided by us, of Jess' late diagnosis and ultimate death dominate page 5 of 32 page strategy. This photo shows an extract. Our girl, and all of you, continue to change things, creating a more positive and safer future for everyone.
