End Crisis of Emotional Breathlessness for over 7​.​5 Million People

The Issue

Chronic respiratory diseases (CRDs), primarily asthma and COPD, affect over 7.5 million people in the UK — that is, 1 in every 8 of our neighbors. Recent epidemiological study confirms that asthma, COPD, and interstitial lung disease (ILD) affect over 10 million people across England, Wales, Scotland, and Northern Ireland, with a substantial number of cases going undiagnosed during the pandemic. Despite this burden, only about half of these patients are receiving appropriate treatment (Whittaker H, Kramer Fiala Machado A, Hatam S, et al. Thorax Epub. doi:10.1136/thorax-2024-222699).

Zenodo. 2025. https://doi.org/10.5281/zenodo.17644275) a staggering 7 in 10 people are unaware of the biological connection between the mind and the lungs. This "knowledge gap" is a clinical emergency. For those living with COPD, Asthma, Pulmonary Fibrosis, and Bronchiectasis, breathlessness isn't just a physical symptom—it is an "Emotional Breathlessness" cycle.


As a respiratory and mental health specialist with over  22 years of experience, I define "Emotional Breathlessness" is a physiological interaction where anxiety and dyspnea reinforce each other. It is not "just worry"; it is a systemic failure of our care pathways that leads to avoidable emergency admissions and stripped-away quality of life.


With global COPD cases projected to hit over 600 million by 2050, the UK cannot afford to ignore the "Mind-Breath" link any longer. Our national survey confirms that millions are left without support because current care focuses almost exclusively on COPD, ignoring the emotional needs of those with other chronic lung conditions.


We call on the Department of Health and Social Care and UK Parliament to adopt the recommendations of the Breath & Mind Report 2025:
1.    National Recognition: Formally recognize "Emotional Breathlessness" as a clinical component of CRD in national care guidelines (NICE).


2.    Integrated Care Pathways: Mandate mental health screenings for all respiratory patients, not just those with COPD.


3.    Specialist Training: Fund NHS-wide training to equip healthcare teams with evidence-based strategies to regulate the emotional-breathlessness loop.


4.    Targeted Research: Commit funding to explore therapeutic interventions for the mind-lung axis.


Every person gasping for air deserves care that supports both the body and the mind. We have the data. We have the proof. Now, we need the policy.
