Aneurin Bevan University Health Board: Restore Public Confidence
The Issue
We, the undersigned, are calling for urgent action to restore public trust and ensure safe, accountable healthcare within Aneurin Bevan University Health Board.
In recent years, a number of serious concerns have been reported in the public domain under the same executive leadership team , including:
Multiple mortuary incidents at The Grange University Hospital, where bereaved families were given the wrong bodies, including one case in which a family cremated the wrong person
A second similar incident occurring within days, raising concerns about systems and safeguards
The case of nine-year-old Dylan Cope, who died from sepsis after being discharged from hospital, where an inquest found missed opportunities in his care and concluded his death involved a “gross failure of basic care” and was likely avoidable
Reports from doctors describing unsafe working conditions and fears for patient safety due to staffing pressures
Welsh Government intervention in 2024 due to concerns about financial management and emergency care performance
Recent media reports that more than 20 patients were treated using unsterilised surgical instruments and were not informed of the incident for several weeks
Concerns raised in the Senedd regarding GP services managed by a private company, where more than 40 doctors were reportedly owed over £300,000 in unpaid wages, alongside wider concerns about service delivery and oversight
These events have caused understandable distress to patients, families, and the wider community, and have raised serious concerns about governance, safety, and accountability.

Healthcare services must be safe, transparent, and effectively managed. When repeated concerns arise, it is essential that meaningful action is taken to rebuild confidence.
We are therefore calling for:
An independent review of leadership and governance within the Health Board
Full transparency regarding patient safety incidents and the actions taken in response. Including the publication of all whistleblowing concerns raised by staff in recent years and by the public.
Clear, measurable steps to restore public confidence in local healthcare services.
This is about ensuring safe care, strong leadership, and accountability for the communities served.
We urge patients, families, healthcare staff, and members of the public to support this call for transparency and improvement.
