The Issue

For many trans women, progesterone is added after estrogen therapy, but its benefits are much less certain than the benefits of estrogen itself. Estrogen is the main feminizing hormone and is responsible for most of the physical changes that accompany a transition, such as breast development, softer skin, reduced muscle mass, and fat redistribution to the hips, thighs, and buttocks. It also reduces body hair growth (along with testosterone suppression) and can bring about emotional and mood changes for some.

Despite the many benefits of estrogen, progesterone can play a complementary role. However, accessing progesterone as part of hormone replacement therapy (HRT) remains a challenge for many trans women in the UK. The restrictive policies surrounding progesterone make it difficult for individuals to access this hormone, which many find essential for their transition. Easy access to progesterone could enhance the feminization process and provide a more comprehensive hormone therapy regimen.

The current regulatory barriers make it a cumbersome and daunting process filled with bureaucratic hurdles and inconsistent healthcare guidance. Trans women are often left uncertain and anxious, not knowing whether they can access the therapies they need to align their physical appearance with their gender identity.

Addressing this issue would not only improve health outcomes for trans women but also promote equality and respect for their identities. Accessible treatment options are a cornerstone of effective healthcare and essential for upholding the rights of trans individuals.

We call upon the UK government and healthcare providers to streamline the processes required to obtain progesterone for trans people. This change can be effected by creating clear, straightforward guidelines for healthcare providers and making progesterone available through NHS prescription. By doing so, the UK will take a definitive step towards upholding the rights and dignity of trans individuals.
