The Issue
California’s current health care system leaves many residents struggling with high costs and gaps in their coverage. Despite spending more on health care than other high income country, the United States continues to see the worst health outcomes with significant disparities across the board, but especially among vulnerable populations. In California, working residents have faced premium increases of 24% since 2022, with an average family’s premiums exceeding $28,000 per year. At the same time, out of pocket costs are projected to nearly double, placing an unsustainable financial burden on millions.
These challenges highlight the urgent need for a more equitable, affordable, and efficient health care system. AB 1900, also known as CalCare, offers a long term solution to address these systemic issues.
AB 1900 (CalCare)
AB 1900 proposes the creation of a statewide, single payer health care system that would provide comprehensive coverage to all California residents, regardless of citizenship status. CalCare would be publicly funded and designed to ensure that everyone has access to the care they need without financial barriers.
Under CalCare, patients would no longer pay premiums, copays, or deductibles. Instead, they would receive a full range of services, including primary care, hospital visits, prescription drugs, mental health care, dental, vision, reproductive services, without the out of pocket costs. Patients would also have the freedom to choose their doctors and providers without worrying about network restrictions.
CalCare aims to reduce health care costs by simplifying the payment system and focusing on actual costs of care rather than profit. It would implement fair global budgets for hospitals and negotiate drug prices, hoping to control excessive spending. By shifting focus back to patient care, providers can spend more time treating patients instead of dealing with billing systems.
The bill also prioritizes equity by addressing health care disparities. It would establish efforts to expand access in underserved communities and invest in health care infrastructure, staffing, and resources where they are needed most.
AB 1900 lays out a structured transition toward a fully funded and operational system. It represents a comprehensive, long term approach to fixing California’s broken health care system, moving away from our current fragmented system towards a unified system that serves everyone.
