I write this petition with a heavy heart, stemming from a deeply personal and devastating loss. My daughter, despite being admitted to an NHS hospital for 9 days, was irresponsibly discharged despite her serious condition. On the day of her discharge, our trust was shattered when the attending nurse failed to accurately document her health conditions, did not communicate key information to colleagues, and revealed a troubling bias.

Neglected in her hours of immense pain,.My daughter was sent to the hospital with 3 slipped discs.Systemic negligence was evident in an untruthful declaration of her fitness for discharge, despite concerns raised by the discharge nurse.My daughter was crying and begging" please dont send me home I am in pain ,"The discharge nurse stated ,patient in wheelchair, condition deteriating , sweating,confused,in pain.My daughter was not allowed back onto the ward .The nurse failed to elevate and documented health concerns. Lack of monitoring, no new bood tests .lack of communication ,Investigation found reaching  the serious  criteria in care putting patients lives at risk. My daughter died 6 hours after discharge.My life will never be the same again  ,but I have to live with a broken heart .The inquest found that my daughter was given 4 different types of prescribed medication that all slowed her heart down .Cause of death Prescribed medication toxicity and copd. 
My daughter is not alone. Many have suffered due to serious breaches of the Nursing and Midwifery Council (NMC) Code of Practice. Measures such as retraining have proven insufficient to handle serious or repeated breaches, let alone when a patient's life is lost due to negligence.

We demand that the NHS implement stricter standards of accountability for all its nurses. Procedures need to be in place to ensure that serious breaches of the NMC's Code of Practice result in appropriate punitive measures, including potential dismissal.
