The Issue

As a reproductive psychiatrist with 25 years of clinical experience, I have dedicated my life to supporting women through some of the most challenging times of their lives including the perinatal and menopausal periods. I founded the Washington Center for Women's and Children's Wellness to advance mental health care for women at every stage in life. Through my practice, I've witnessed firsthand the profound impact depression and anxiety can have on women and their families.

Recently, there has been a proposition to implement an additional black box warning on SSRI (Selective Serotonin Reuptake Inhibitor) antidepressants, suggesting they cause birth defects. This warning is not only misleading but could deter women from accessing necessary treatment during pregnancy. SSRIs are lifesaving for many women; they help maintain mental health stability, which is paramount for both the mother and the developing fetus.

Evidence does not conclusively support the notion that SSRIs cause birth defects. In fact, several studies indicate that untreated maternal depression poses a higher risk to fetal development than SSRI exposure. Research, including reviews published in peer-reviewed medical journals, has consistently found no significant increase in congenital malformations with SSRI use during pregnancy.

It's vital to prioritize science-based information. A black box warning could exaggerate fear among expecting mothers and healthcare providers, potentially leading to the discontinuation of essential medications. This decision could have devastating consequences such as increased rates of postpartum depression, maternal suicide attempts, and disruption of family dynamics.

We urge the FDA to reconsider this proposed birth defect warning and focus on disseminating balanced information concerning the use of antidepressants during pregnancy. Education and informed decision-making should be at the forefront of this issue.
