To err is human……
But to persist in error is diabolical.
Mistakes will happen, humans are fallible.
Making sure a mistake does do further harm is key.
Correcting and learning from the mistakes is priceless.
Improving the system without mistakes is ideal.
1): Communication, Communication and Communication
Recognize it’s importance.
Understand the need for clear instructions and
have back up in place, to avoid a miscommunication.

Ask questions, especially if you not sure or unsure.
Importance of having a family member or loved one engaged on your behalf.

Healthcare is a team process.
A patient’s health is everyone’s responsibility-
All Healthcare providers, the family and the patient must be on the same page, working together.

Learn to deal with mistakes in a practice environment.
Simulations before it occurs.
Recognize and identify potential problems.
Worse case scenarios. Look for the holes.
In case of emergencies, what do we do next.
Mistakes: Questions and answers in training rather than in real life cases.

5): Allow a system of medical error reporting that is consistent and conducive to maximal transparency first. A Rapid response to errors to prioritize the patient and treatments to avoid further harm.
Review and investigate using experience and science.
Remove scapegoating and blame.

Arbitration, mediation over litigation.
