Joel Cauchi murdered Yixuan Cheng, Faraz Tahir, Jade Young, Pikria Darchia, Dawn Singleton and Ashlee Good at the Bondi Junctio  Westfield Shopping Centre on April 13, 2024, 
Yixuan, Jade, Pikria, Dawn and Ashlee were among 105 women lost to violence in 2024. Their killer deliberately targeted them and other women throughout the massacre. Faraz - a security guard - died trying to save lives.
The NSW Coroner has handed down her findings following the inquest into their deaths, finding the shrink responsible for the killer’s care made a major mistake that could have prevented his actions.
From 2012 to 2020, Cauchi was a patient of Queensland psychiatrist Andrea Boros-Lavack. He had severe schizophrenia. She weaned Cauchi off his medication.
The psychiatrist "did not adequately monitor him for early warning signs of relapse".  
The coroner has recommended the regulator review the psychiatrist’s treatment and care of Cauchi; the the development of guidelines relating to the care and management of people with schizophrenia; and  urged the NSW government establish short term accommodation for those experiencing mental health issues and homelessness.
Throughout my years documenting femicide in Australia, I’ve found a long list of systemic failures. 
In every femicide, there is a point where her life could have been saved. Holes in our safety nets allow violence to thrive.
Without a complete and exhaustive national examination into the killing of women and girls, we will never be able to understand or respond to the flaws and failures that keep fueling the epidemic of violence in our country.
