Require accountability and transparency from the NHS
The Issue
I want to highlight something that affects us all, the failing health care system.
In July of 2025, my mum underwent an operation to remove her kidney. During this procedure, her bowel was perforated, and a needle had been lost for 45 minutes of an operation that lasted over 11 hours.
Subsequently, my mum allmost went into cardiac arrest due to the trauma her body went through, she contracted hospital-acquired pneumonia and a blood clot in her lung, all of which have severely impacted her health and each day is an ongoing battle, my mum has now had to return to hospital on 4 times within 7 months and there is no sign of improvements, we have been told that if there can be no surgical intervention, my mum will be placed on pallative antibiotics.
The doctor who performed the surgery is still practising and even has a private practice, my mums life has been devastated by what has occured and yet it has had very little impact on the people who have failed her.
We were told to raise a complaint, which we have done 3 times and months later we are still awaiting a response to unanswered questions.
Alarmingly, we've discovered that her bowel had been perforated previously at a different hospital in 2023, yet we were never informed of this potentially fatal error. Unfortunately, my mum has not made a full recovery.
This demonstrates the NHS are a law unto themselves, the doctors and surgeons do not feel they should be held accountable or even disclose when they have made mistakes.
This experience has shown me the devastating consequences of a lack of accountability and transparency in the healthcare system. Unfortunately, this issue is not unique to my family. Many other patients and families endure similar distress due to the failure of medical professionals and institutions to admit when mistakes occur and to communicate these errors effectively.
The NHS system operates with minimal mandatory transparency and accountability measures. We have followed the process of raising a complaint without any response, we are now demanding answers from NHS greater Glasgow and Clyde region health board.
We must hold doctors, surgeons, and healthcare institutions accountable for their actions. This petition calls for legislation that requires mandatory reporting of medical errors to patients. Hospitals should also be legally obliged to disclose any mistakes that occur during a patient’s treatment. In doing so, trust can be rebuilt between patients and healthcare providers, and improvements in patient care and safety can be achieved.
My mum has been in hospital for the first two weeks of 2026, they gave my mum medication that we had raised concerns about, this resulted in my mum taking a severe reaction, she subsequently took a dizzy turn, fell and hit her face and head.
They are stating that they can no longer provide care to my mum in a community setting and if my mum discharges herself, she will be placed in palliative care, my family are heart broken, they failed my dad and now they've done the same to my mum.
We propose the following actionable steps, this should become legislation:
1. Implement a national legal requirement for immediate disclosure of any medical errors to patients and their families.
2. Establish a process that investigate reports of medical mistakes in a timely manner and ensure compliance with new regulations that a doctor must not be allowed to continue to practice in an NHS hospital, whilst under investigation for serious medical negligence.
3. Mandate comprehensive training for healthcare providers on how to effectively communicate errors to patients and learn from these incidents to prevent future occurrences.
4. Doctors must also have a clear understanding of key events in a patients medical history.
5. Doctors should disclose if there are any complaints or investigation, of a serious nature against them.
These initiatives will drive a culture of transparency and accountability in healthcare, reducing preventable harm and restoring public trust.
