Maine recently adopted new regulations that could dramatically reduce access to outpatient IV therapy for patients statewide. They are some of the most restrictive in the country.

We fully support regulations that protect patient safety. However, two new requirements go far beyond the standards used in most healthcare settings and could make safe, convenient IV therapy much harder to access.

What's Changing?
The new rules require physicians to personally prepare many IV infusions before they can be administered, rather than allowing Registered Nurses to prepare them under provider standing orders.

Registered Nurses have safely prepared and administered IV medications under provider oversight for decades in hospitals, infusion centers, emergency departments, and physician offices. They are extensively trained in medication preparation, sterile technique, IV therapy, patient assessment, and emergency response.

Requiring physicians to personally mix IV infusions creates unnecessary barriers without clear evidence that it improves patient safety.

Why This Matters
If these rules remain in place, patients across Maine could experience:

• No more walk-in IV therapy
• Longer wait times for treatment
• Reduced access to care, especially in rural communities
• Higher healthcare costs
• Fewer locally owned healthcare options
• Many small, nurse-led clinics may struggle to continue operating under these new requirements.

What We're Asking For
We respectfully ask the Maine Board of Licensure in Medicine and Maine policymakers to:

• Allow Registered Nurses to prepare IV infusions under provider or advanced practice provider standing orders.
• Remove the requirement that physicians personally prepare IV infusions.
• Align Maine's regulations with evidence-based healthcare practices while maintaining patient safety.
