Here are some examples where Child Protective Services (CPS) failed to act, prioritizing the institution's reputation over the safety of the children:

1. **Case of a Seven-Year-Old Child:** A seven-year-old child in a mental health institution exhibited multiple injuries and signs of abuse. The child's family reported the abuse to the Oklahoma Department of Human Services (OKDHS). Despite the detailed reports, OKDHS conducted a superficial investigation. The child remained in the same facility, and no substantial actions were taken. Internal communications later revealed that the institution's reputation was a concern, leading to inaction.

2. **Institutional Cover-Up:** In another case, a nine-year-old child was admitted to a mental health institution. Over several months, the child displayed visible signs of physical and emotional abuse. Multiple reports were filed by concerned staff members and the child's family. However, the institution's administration intervened, downplaying the severity of the abuse to avoid bad publicity. The OKDHS investigation was delayed, and the child continued to suffer abuse without intervention.

3. **Ignored Reports:** A ten-year-old child in a different mental health institution experienced severe abuse. Reports were made to OKDHS, but the department's investigation was superficial and inconclusive. Internal emails later showed that the institution's management was more focused on maintaining their accreditation and avoiding legal repercussions than addressing the abuse. As a result, the child's suffering continued without proper intervention.

These examples highlight the failures of CPS to adequately protect children in mental health institutions under current laws
