The Issue
We need your help to join us in our call for a Public Inquiry into Tees Esk and Wear Valley NHS Foundation Trust (TEWV).
TEWV NHS Trust have failed in its duty to provide patients with safe care.
Within a 5 year timeframe, at least 10 of those below, lost their young lives, whilst in hospital, under the care and treatment of staff. A number of those were Autistic.

April 2024 - CCQ Prosecution as TEWV plead guilty for failing “to provide safe care and treatment” which exposed patients to “significant risk of avoidable harm.”
The Care Quality Commission (CQC) have successfully prosecuted Tees, Esk and Wear Valley (TEWV) NHS Trust for the failures in the care of Christie Harnett and Patient X. Following the sentencing at Teesside Magistrates Court on the 18th of April 2024, TEWV has been fined £200,000.
“TEWV is among the most concerning mental health Trusts, with a series of preventable deaths in their care, as was the case in Essex before the inquiry was announced in November 2020”
Background information
We are a collective of families who have suffered the most painful losses, the deaths of our loved ones , whilst they were in the care of professionals, who were meant to be caring for and keeping them safe.
We don’t want any more families to suffer or to lose anyone else through inadequate care and preventable deaths.
As you may know, NHS Essex Partnership University Foundation Trust have got their statutory public inquiry after years of fighting. We stand with them united as families failed by TEWV Trust.
The charity INQUEST, The Northern Echo and The Rebuild Trust support us into calling for a statutory public inquiry into deaths and serious incidents in mental healthcare.
Between October 2008 and October 2015, a period of seven years, there were 310 deaths of patients reported by the Trust.
In comparison, in the seven years which followed, October 2015 to January 2022, there were 753 deaths of patients reported by the Trust, an increase of over 142 per cent.

Sadly these stories of such preventable tragedy are not uncommon.
There have been countless failings identified and the CQC have recognised many services within the Trust as INADEQUATE
Lessons have NOT been learned, as the same mistakes are being repeated time and time again. Nothing has changed,
