The Issue
In May 2025, The Lactation Network (TLN) made the difficult decision to stop submitting claims to Blue Cross Blue Shield (BCBS) of Alabama and several other BCBS plans due to inconsistent and unsustainable reimbursement rates. As a result, many families in Alabama have lost access to high quality lactation support that is provided by International Board Certified Lactation Consultants (IBCLC) in an outpatient setting.
Currently, BCBS Alabama does not recognize IBCLCs as independent, billable providers—unless they are working under a physician or hospital. This policy has left countless new and expecting mothers without affordable or accessible breastfeeding support during one of the most vulnerable phases of motherhood.
Breastfeeding is not just a personal choice—it’s a vital health decision. Research shows that breastmilk helps build a baby’s immune system, supports brain development, and promotes long-term health. For mothers, breastfeeding fosters bonding and improves mental health, while also lowering the risk of certain diseases.
IBCLCs are the highest-trained professionals in lactation care. They help mothers overcome common challenges such as latching difficulties, low milk supply, pain, and more. When these providers can’t bill insurance directly, many families are forced to either pay out of pocket or go without this critical support—putting breastfeeding success and health outcomes at risk.
The inability of IBCLCs to bill BCBS Alabama directly, especially when they’re not affiliated with a hospital or physician, is a major barrier to care. This must change.
We’re calling on BCBS Alabama and other insurers to:
- Recognize IBCLCs as independent healthcare providers in order to directly bill BCBS for lactation consultation services.
- Restore coverage for out-of-network lactation care, including partnerships with trusted services like The Lactation Network.
- Ensure fair and livable reimbursement rates for IBCLCs, so they can continue offering the expert care families rely on.
Every mother deserves access to expert breastfeeding support—no matter where she lives or who her provider is. And every IBCLC deserves to be fairly paid for their work.
