As promised, attached and in-line, includes the thank you follow up email to you and [redacted] and the formal request you both asked me for so that a letter addressed to the Department of [redacted] can be written and picked up, that specifically outlines what was in my complaints and grievances to the Coalition of the [redacted] earlier this week and the Ombudsman’s office, in person, on October 31, 2024.
This formal request discussion in her office , that I submitted in writing (since no advocate came to the meeting) is one of many issues in both of my submitted written complaints including the copy I gave you of my complaint to the [redacted] coalition. and to the Ombudsman on the 31st.
Both Complaints from last week to the [redacted] and the complaint that was submitted to the ombudsman are inline and attached . May 11th, 2023 attachments are not uploading. I will send those in a separate email and printouts.
You said you have not been receiving the copies that I have cc’d to[redacted] , however perhaps there is another place that she has left them for you. I emailed you the November 4th and November 6th recertification[redacted] ,[redacted] recertification application proof documents for my[redacted] to renew and the phone interview details along with my taxes from April 2024 and my birth certificate affidavit, for you to print out for me on November 20, 2024.
I understand you did not have enough paper to print them out for me to take to [redacted] on November 20th, however you did see me upload them and was present when I called Access[redacted] for their verification over the phone regarding my missing CIN number in the[redacted] system and verification of my uploaded documents that day since we could not submit hard copies due to your printer not having adequate paper.
Ms.[redacted] is very much on top of these things. Please look again. Every correspondence including emails with dates and headers have been printed and left for you. I will be sure to send the hard copies from today’s email correspondence, as well.

Thank you very much for taking the time to meet with me to submit my formal complaint, which has been forwarded to the Ombudsman for [redacted] and[redacted] . I am writing to formally request that the staff at[redacted] , including the [redacted] Director and Program Manager, provide a letter to the Department of[redacted] and [redacted] confirming that I have never been diagnosed, evaluated, interviewed, or established any form of patient-doctor relationship with any [redacted] [redacted] staff members at this organization, whether directly or indirectly.
Additionally, I would like to request the removal of any documentation in my file that was written by a case manager named [redacted] or any other staff member, including[redacted] . This documentation is unrelated to my housing search, and I do not consent to having it in my records.
Please be advised that I have no history of [redacted] nor do I have a history of[redacted] abuse. I do not use[redacted] , do not[redacted] , and have never been an[redacted] . I also have asthma, a condition that significantly affects my ability to breathe, especially in the presence of smoke. I cannot allow smoking within certain proximity of me due to the severe health risk it poses to my asthma. Therefore, I do not want any documentation regarding these matters in my records. Furthermore, I request that no information that violates my protected HIPAA rights, whether by contributing to or taking information from my records, be included in my file.
I would also like to address the visit to my room on Wednesday, November 27th, by you, your[redacted] staff, and your[redacted] , to verify whether or not I was smoking in my room. 23 Locust Street is an apartment I searched for, found, and settled on with the landlord. I rode in the landlord’s car to see the apartment, visited a second time with a broker, and was promised to move in on September 1, 2024. Once I provided the[redacted] [redacted] and[redacted] [redacted] with the landlord’s contact information, the landlord received a mysterious call claiming “Ms. Arms did not want the apartment.”
When I asked the landlord to provide phone records, his office went silent, and so did the [REDACTED] and [REDACTED] manager. No one has responded to my numerous requests to obtain both[redacted] records and the landlord’s phone records, dates, or times of the alleged requests and actions.

I requested[redacted] to provide me with IP address and device information regarding who challenged an appeal on my behalf after my[redacted] [redacted] created my username and password and asked me to allow him to handle all correspondence and requests from[redacted] . My request was in response to a[redacted] denial due to “zero income” and someone secretly misrepresenting me in this so-called appeal, which must have been submitted by[ REDACTED] specialist [redacted] and[redacted] [redacted] [redacted] .
The false narratives around these[redacted] rules were created solely to sabotage my ability to obtain[redacted] and force me to leave the[redacted] under “violations” that I never engaged in.
[redacted] Fraud and Falsification of Records
I have requested to reschedule two meetings with [redacted] manager[redacted] since entering the[redacted] with the[redacted] in November 2022 and here at[redacted] since December 2022. With the exception of being on an eight-hour call to[redacted] Access on the day of my[redacted] meeting in early 2023 and a missed[redacted] due to illness from a non-working heater, I have never missed or re-scheduled any other[redacted] meetings.
Since my arrival, there have been administrative personnel problems due to a lack of adequate leadership and a law enforcement/private investigation team that manipulated incidents and intercepted over 1,000 emails on SMTP email servers and phone calls, including the intercepted, altered proprietary[redacted] Access system. This included returned phone calls regarding my housing that I secured for myself, any calls made to agencies and the[redacted] Coalition, and even calls made to my private attorney when certain members of legal-aid began colluding with them.
There was interception of an initial emergency [redacted] on May 11, 2023, which had been initially approved by the case manager during the interview. (See supporting documentation).
Immediately following the brief application, interview, and submission of all requested documents, on May 11, 2023, the case manager,[redacted] , gave me a letter stating she was the acting Program Director while[redacted] was in an office stating she was also the Program Director.
[redacted] also falsified in a document, after that normal[redacted] application [AND] brief meeting, that “although undiagnosed,” I had [REDACTED] issues. She improperly wrote down diagnoses and a summary of what she believed those diagnoses should have been.
While[redacted] reprimanded[redacted] in an email (now missing from servers for two email providers) for placing those statements in my binder and scheduling my ILP meeting on the day of her child’s graduation deliberately[redacted] followed up by inappropriately assigning me to a [redacted] as my next[redacted] .
Administrative Changes
[redacted] and another [redacted] manager who witnessed [redacted] actions resigned, along with many of[redacted] staff members who opposed and witnessed the infractions.[redacted] , the organization’s Vice President, also the organizations attorney, and founder[redacted] devised a plan for[redacted] to retire, and a new administration was put in place while my letters to [redacted] and Program Director [redacted] went unanswered. I was then assigned a new case manager and experienced a revolving door of assistant PMs and PMs being briefed by none only than[redacted] , or those coached by[redacted] .
I also informed all parties that my domestic violence situation, combined with organized crime, caused me to approach the[redacted] for assistance with[redacted] . I requested placement as a professional working woman with no personal hygiene issues, a lifetime of being drug-free, and 100% mental strength, with no history of mental illness.
I had a registered business out of state and was still teaching for another registered business where I had been employed for over a decade. However, I struggled to provide specific paystubs and did not know my newly issued Social Security number and could not obtain it without an address. Being stranded here after a car accident and the pandemic forced me to have to enter the [redacted] to obtain my social security number and other important Identification. However, upon arrival at the shelters, I submitted copies of my articles of incorporation and EIN number, requesting they use this information to contact the Social Security Administration.

I met with Ms.[redacted] for my next few[redacted] because I felt it was suspicious and inappropriate for this actions considering my repeated requests for these defamatory actions to cease and requested a new[redacted] manager.
Ms.[redacted] and another case manager that witnessed what Ms.[redacted] was doing, she resigned along with many of her staff members who opposed and witnessed the infractions, and[redacted] [redacted] , the organizations’ attorney and Dr.[redacted] devised a plan for her to retire and a new administration was placed, assigning me a new case manager and a revolving door of assistant PM and PM.
I read on the[redacted] website that the facility was transitioning to a[redacted] [redacted] [redacted] center and noticed new clients coming in that fit the description and began writing letters to Dr.[[redacted] ] [redacted] and the staff attorney. I was directed to Program Manager[redacted] [redacted] and also[redacted] [redacted] , in which either they never responded to my calls and emails, or the communications were intercepted.
In each of my meetings and correspondence, I informed her that I was a professional Investigative Journalist and Broadcast Engineer that has been a community leader and educator that had to escape my situation from walking away from my home and starting a new.
-Failing to submit bank statements and income information [AS SUBMITTED TO CS]
I also informed all parties that my domestic violence situation combined with “Organized Crime” caused me to approach the[redacted] and asked if they could assist with placement of a professional working woman that has no personal hygiene issues, never used drugs a day in my life (Lifetime drug-free), and[redacted and corrected]% mentally strong with no history of[redacted] [redacted] and that I had a registered business out of state that had just started operating, but I was still teaching for another registered business that I had been teaching for over a decade.
While I did not have specific paystubs and did not know my newly issued social security number, I had copies of my articles of incorporation and my EIN number, that I gave them a copy of to reach out to the social security administration to obtain as it was tied to my EIN.
I have repeatedly requested what W2 checks did the [redacted] require? I have placed Paychex and ADP on hold for months attempting to get a straight answer regarding W-2 income over filed [corrected] taxes. I reached out to NELP at the Brooklyn Chamber of Commerce to assist with entity formation changes to an LLC so that I can issue myself a W2 paycheck, if that was a requirement.
Ms.[redacted] and every[redacted] manager after said I needed to apply for[redacted] [redacted] [redacted] . I did not want to be in violation of local, city, or federal laws regarding owning a business and being on[redacted] [redacted] so I requested proof that I was required to apply for[redacted] [redacted] and proof of the rules regarding specifics W-2 income and whether or not I needed to put aside money for my apartment. I don’t really have a great deal of money to put aside for payroll, however I have repeatedly been told to apply for[redacted] .
During each[redacted] that[redacted] showed up for, which was not many, she missed the majority of our[redacted] meetings going to her child’s graduation in which just a day prior, she confirmed our meeting, the next meeting she went out to lunch during our scheduled meeting and scheduled visit to[redacted] street[redacted] office and I left a note on her seat and asked her co-worker to let her know I was still here after waiting outside her office for over an hour, she never returned nor took me to[redacted] Street to see why the system kept saying that I never submitted the remaining documents.
I personally went to[redacted] street alone, when Ms.[redacted] suddenly stopped showing up for meetings and I would find myself waiting on the “C” level for three hours or more waiting to meet with her. The very last meeting she attempted to make me look like I had the meeting date and time mixed up and printed a new notice with an incorrect date on it. I asked her to reprint it and showed her our originally scheduled meeting notice and she tried to snatch it from me and requested I give her back the notice. I told her I was retaining it for my records and filing a police report.
A few weeks after requesting to see a supervisor at [redacted] Street, in which I retrieved a receipt ( that was taken from my room by someone with access keys), and requested a review, I also re-submitted my documents, the agency refused to co-operate and never opened the case nor issued any emergency[redacted] and then mailed me a letter stating it was because I did not have a birth certificate, a document I requested that[redacted] help me obtain, and one in which she stated she requested. She never assisted with my birth certificate, although I had submitted school records , received FASFA for Kenzie Academy, submitted a valid drivers’ license, and submitted my social security card along with my school records from the MEOC/BMCC to verify my citizenship. I have since submitted a birth certificate affidavit from my estranged biological father that I have not seen since 1978, but who could verify my birth in an affidavit and in an application for a birth certificate in which he received assistance with through a librarian out-of-state since he is elderly and living in a nursing home. I submitted that affidavit with all of requested documents for re-certifying my[redacted] [redacted] . Even though, Yasheesh, the interviewer for[redacted] [redacted] said NONE of identity documents were needed, just my taxes, since that was the only change since my[redacted] was opened up. I suspect[redacted] was involved in the request of my identity documents following that interview since she was the only who had knowledge that I needed the birth certificate since she herself never assisted me with obtaining it. She should be terminated for her dishonesty.
Each and every interaction following each new administration, which has been briefed, coached, and coerced, by[redacted] has resulted in my showing up to every [redacted] to be falsely accused of being in violation and me refusing to agree with the falsified one-sided document in which I indicated on each and every one and was assured by[redacted] , the now supervising[redacted] director over ALL case-management, that the statement on my[redacted] “did not really apply to my situation, it is just automatically printed on each [redacted]”.
My new [redacted] manager[redacted] and I have been successfully meeting weekly, then bi-weekly and then she requested for us to meet monthly , however we are now meeting in December for the first bi-weekly meeting since August, again because she was not granted her request to meet with me monthly . We both agreed that since I am housing ready.
Lastly, I was given my first apartment application, by[redacted] , [redacted] on November 20, 2924 and worked the entire week last week remotely waiting for the car that she said she would call for us to see the apartment, I was also denied for another[redacted] housing connect apartment last week. While home last week, I received a phone call from my[redacted] that said The [redacted] called her and told her that she emailed me and said she needed me to sign a lease,[redacted] my [redacted] to let me check all of my emails as I was logged in directly to the email servers and NOT using an email app, I confirmed with my[redacted] that no such email came in and that I was taking screenshots and I also informed my [redacted] that no visit to the apartment had taken place yet and that I couldn’t sign a lease until the[redacted] took me to view the apartment. As of today, November 26, 2024, I have called and spoke to the[redacted] supervisor and left messages for this [redacted] and her supervisor and to date, I have not received a return call.
The letter from this [NEWLY ONBOARDED] [redacted] Manager is the only acknowledgment I have received after requesting the[redacted] violation be removed from my records , a request for all of my records, especially , unauthorized evaluations , observations, assessments from people I requested to not[redacted] me or contribute to or access my protected HIPAA information.
There has been mail fraud and stolen financial documents from my room, that relates to missing TD bank deposits, transactions, bill payments, filed grievances and appointment letters,[redacted] letters, and a stolen and re-ordered[redacted] [redacted] card for me and stolen[redacted] [redacted] from another resident that believes[redacted] was the only one that knew their social security and [redacted] information for those benefits to be stolen and that resident asked me to contact the[redacted] center, the police, and the Inspector General, in which I did, on their behalf.
