Watchdog reports have detailed dangerous conditions at Bridgewater State Hospital in Massachusetts, including patient abuse, illegal restraints, and severe health hazards. Despite calls for reform and reports spanning more than a decade, the facility, which houses individuals with mental illnesses, has continued to face serious concerns.
Patient abuse and neglect Involuntary medication: Reports describe the use of force to inject sedatives into unwilling patients, often when there was no emergency. Staff, sometimes in riot gear, would physically hold patients down to administer medication.
Illegal and traumatic restraints:
The Disability Law Center has repeatedly found that staff use restraint and seclusion, including chemical restraint, illegally and excessively. These practices are reportedly used as punishment rather than for clinical needs.
Physical violence: The DLC has documented instances of physical violence from staff, including one 2022 video of staff pulling a patient back into a room by his leg and piling on top of him to administer an involuntary injection.
Fatal restraint: 23-year-old Joshua Messier, who had schizophrenia, died after being restrained by guards. A video showed guards pressing their knees into his back, a forbidden technique. The medical examiner ruled his death a homicide, but no criminal charges ultimately resulted in conviction.
Hazardous physical conditions Widespread mold: Extensive mold has been found throughout the hospital, including in the medical building and HVAC system. Testing in 2019 found contamination in nearly every swabbed area.
Facility deemed uninhabitable: The mold issues have been so severe that experts have stated the building should not be occupied by patients or staff until the problems are resolved.
Negligent repairs: Despite attempts at mold remediation, advocacy groups have stated that the work was not completed to industry standards and that no independent review was conducted to ensure the facility was safe.
Inadequate medical care Negligent care: Advocacy groups have alleged negligent medical care and insufficient access to clinicians and therapeutic programming.
Inaccessible services: It has been reported that the process for accessing medical services is not accessible to patients with disabilities or limited English proficiency.
Persistent oversight failures DOC oversight: Bridgewater State Hospital is under the oversight of the Massachusetts Department of Correction, rather than the Department of Mental Health, despite primarily housing individuals with mental illnesses.
Unfulfilled promises of reform: Calls to transfer oversight have been made for years.
An external healthcare company was brought in from 2015 to 2017 to improve conditions, but recent reports indicate that the benefits of that effort have faded.
Ongoing legal violations: Recent reports from 2024 and 2025 confirm that illegal practices and deficient care have continued despite previous investigations and legal settlements.
