I want to share a personal update. Like thousands of others, I was recently switched from Levemir to Lantus. Tonight will be my first dose.

I was simply prescribed 14 units of Lantus — no explanation, no adjustment plan, no warning.

But here’s the reality: Lantus is not a biosimilar for Levemir. These two insulins are different. They may both be long-acting insulins, but they don’t act the same way in every body.

⚠️ The American Diabetes Association (ADA) even recommends reducing a patient’s total daily insulin dose by about 20% when converting from Levemir to Lantus, as a precaution against hypoglycemia. The dose should then be carefully adjusted based on glucose response.

That didn’t happen for me. I wasn’t told any of this — I was just switched.

Lantus also carries an increased chance of nighttime hypoglycemia. I know firsthand how dangerous that is: just this past weekend, while still on Levemir, I dropped to a blood sugar of 50. If that can happen on the insulin that worked best for me, what will happen now that my only option is one with a higher risk of lows in the middle of the night?

This isn’t about convenience. It’s about safety.
It’s about choice.
And it’s about the fact that patients are paying the price for decisions made without us at the table.

We deserve:

Transparency from manufacturers.
A real transition plan — not “just switch and hope.”
Affordable, accessible alternatives for every patient, not just the lucky ones who tolerate the substitute.
