My mom was in this facility for physical therapy so she can regain her strength back before her return to her apartment. Not even 2 weeks, I got a call from the nurse to tell me that my mom fell down twice that I know of and she needed to go to the hospital for further observation, since she was on blood thinner. I stated to the nurse at the facility that my mom has on a (FALL RISK BAND) , someone should have been watching her. Upon arrival at the hospital, I observed her fall risk band was taken off of her before she came to the hospital. That should have given me a red flag! However, I spoke with the nurse director and she promised me that they will supervise her more closely.

When my mom returned to the facility, I noticed changes in her mental status. I spoke with the nurse who was assigned to her and stated that they were still giving my mom medication for pain that was discontinued in the hospital before she returned back to them. I observed the staff demeanor toward other staff as well as the family member. The nurse/ staff behaviors were very inappropriate. My mom had three appointments' while she was there and I attended each of them.

On 11/6/20, the facility called me to remind me that my mother had a follow-up Cardiology appt. with her NP. I went to the hospital and waited for the ambulance to transport my mom to her appointment. After being there for more than 3 hours, they never showed up. I called the facility and was told that it was in traffic. The NP was waiting for a while, she also called and they told her they went to the wrong hospital. So the appointment was cancelled. I called the ambulance company who transports patients to their appointments and said that they remember my mom, and reported that the facility cancelled her appointment on that same day at 12:30 pm.

On 11/9/20, I received a call from the facility that my mom was on her way to the hospital, because she didn't have a pulse. On arrival at the hospital, I was told my mom passed away and now I am planning her funeral. If she would have made it to her cardio appointment, maybe her NP would have seen if she had anything going on with her. For some reason the facility avoided her to go to her appointment on 11/6/20. Also, I received my mom's Medicare report in the mail, and noticed the doctor visited on that same day at 5pm, the same same day she missed her appointment. 
My mom had a pacemaker, and the purpose of the pacemaker is to help your heart beat more regularly. So how can you explain that she was unresponsive? I was told she was in the bed unresponsive,  and 5 minutes later another staff member observed her with no pulse. Per EMT report, who arrived at the scene stated that my mom was on the floor , head up with white secretion coming from her mouth. Also, when I reviewed her body at the hospital, she had dried blood behind her head. No one examined her body in the ER. I have photos with the dried blood and she also had bruises on her lower extremities.

In addition to the above , I was talking to the social worker about discharging my mom back at home that same week. Instead my family and I had to plan her funeral. This is very devastating especially when you read the reviews about this facility and other family members who went through the same heartbreaking news about a loved one. This is a place that has a reputation of abusing elderly patients, lack of staff, and mistreating them. Also, I observed the nurses' attitudes, no interaction with the patients and the smell of the place is substandard.  

I believe the nurse should be held responsible as well. My mom was on NPO ( nothing by mouth) I saw on the nursing notes that she was given medications by mouth, which could have explain the white secretion from her mouth. Most likely , she fell / aspirated.
