By signing this petition you are attesting that you are a practicing licensed dentist in the United States that files CDT code D9211 regional block anesthesia with restorative or surgical procedures on third party claims when providing this service.  You are also attesting that if this code is not covered by third-party payers that you pass the fee for this procedure onto the patient; you do not consider this code to be "local anesthesia" and thus not bundled with restorative or surgical procedures.
