Our Dad Michael Daly passed away 7th April 2010, his death was untimely, we called the Gardai to the hospital when our Dad passed away and asked for an autopsy and an inquest into his death as we had become suspicious about the care he did and did not receive.

Because of the testimonials of others at Inquest 2012, Limerick Coroner at Inquest had to call a verdict of HEART FAILURE NATURAL CAUSES  due to being given misleading information as we now know.

We disagreed with this verdict and I set about investigating our Dad's death and after 4yrs I compiled an overview of what I say happened our Dad which was no where near the offical cause of death we had been given, this was backed by international experts and upon receipt of their reports.

This was 2017,  I then contacted the Coroner of Limerick and after receiving some documents from university hospital Limerick that was not available to us at the 1st Inquest that I showed the Coroner he agreed to a 2nd fresh new Inquest.

This took a few years as the Coroner brought in his own experts, as well as Covid period, so eventually the second inquest began March 6th 2023.

Experts at inquest were,   a Colo Rectal Expert,   An  Intensivist Expert,  a Radiologist Expert,  and former retired State Pathologist Professor Marie Cassidy

FINALLY - on March 9th 2023   the TRUTH of why our Dad died came out at the Inquest. 

Verdict  MEDICAL MISADVENTURE 

Which overturned the verdict of 2012 of natural causes......

This alone is worrisome,   that an inquest in 2012 resulted in natural causes,  yet overturned in 2023 with medical misadventure.

An inquest is a place where no blame is given,   it's about  who why where when and how, the death occured. 

The bigger story of facts need to be heard in the interest of public safety 

What happened our Dad was horrific, and is unimaginable and will scandalise the nation.

This is a horrifc and deeply disturbing case that raises serious questions about the medical treatment provided to our Dad and the circumstances surrounding his death.

The fact that this was the first time in the history of the State that a second inquest into the same death has been held underscores the importance of a full and transparent investigation.

The fact that a second inquest has reached a different conclusion than the first, and that new evidence has been uncovered, is alarming and demands a thorough investigation.

I urge the relevant authorities to launch a comprehensive investigation into this case and to ensure that all necessary steps are taken to prevent such tragedies from occurring in the future.

We, The Daly family deserve answers and justice, and the public deserves to have confidence in the medical profession and the healthcare system. 

We, the Daly family call upon the Irish government to commence a FULL PUBLIC INQUIRY into the care and death of Michael Daly Snr (Deceased)
