The Rise Associates report found many failings and this was reported widely.  https://www.independent.co.uk/news/health/nursing-midwifery-council-review-regulator-b2574863.html

“We spoke to several people who argued that drawn-over investigations were a contributory factor to six nurses taking their lives in the last year,” the report authors wrote. “We also saw correspondence from a mother who directly blamed the NMC for her daughter’s death because of an incompetent and biased investigation.”

"The authors said there was “dangerous groupthink” at the NMC and highlighted how leadership did not take safeguarding warnings seriously. They said too many staff were “struggling” and were “angry, frustrated and exhausted”.

One matter was often bad nurses are not the ones being investigated. "Jason" ( name changed for anonymity ) was referred by  a manager who was most likely responsible for poor care and yet still works unrestricted and to date has not had any investigation opened on them

“Good nurses are finding themselves being investigated for years over minor issues and bad nurses are escaping sanction because of a system that’s not functioning as well as it should.”
