We were contacted by Paul who went through FtP 2 years ago with conditions of practice applied to his PIN, after which his case was closed once the conditions were satisfactorily fulfilled

He was demoted on his return to work, but relieved as this meant he could return at his pace without potential triggers of a more stressful role.

He is close to retirement and so aimed to just work his last few years, quietly until he could officially retire

This week Paul was informed he had been referred to the NMC again - he was distraught and tried to take his own life

We have escalated to the NMC safeguarding team to ensure measures are put in place during their liaison with him - however Paul had mental health issues during the last referral including involvement with the local mental team following a suicide attempt.

We are shocked that there was no flag to highlight this with extra care taken when informing him of his new referral to minimise risk where possible. 

This is one of the many safeguarding issues we aim to address with the Nursing and Midwifery Council
