Adele Collins & Fabiana Palhares died six days apart. They both turned to police and other services for help before allegedly being murdered in domestic violence attacks.
It has devastated family and friends who are left asking where did it all go wrong. Since January 1st, 14 women across the country have died and counting.
Domestic violence is increasingly 'on the agenda' – yet governments aren't acting decisively to fix the system that's seeing women die. That's what domestic violence death reviews would finally do. It's time we review every domestic violence death and act on the findings.
We have both worked in domestic violence and sexual assault services for a most of out working lives. We have seen first-hand the devastating impacts these deaths have on family and friends who lives are ripped apart by the loss of those they love.

We know from expert studies that this strategy works. That domestic violence death reviews – that are cross-government & community, transparent and highlight system failings – can begin to turn the tide on these homicides.
Domestic homicides and breaches of protection orders have increased in Queensland and we believe Premier Annastacia Palaszcsuk has the chance to leave an incredible legacy by ending this surge in domestic violence deaths.
