Says he feels of "off" today and doesn't know why

Today, I took my son out to dinner so we could hang out and laugh after such a heavy day yesterday. He said to me that he felt off today and didn't know why. He said he was extremely quiet. I didn't want to ask too much, but my heart broke. I am seriously disgusted.
