NYS CCO LIFE PLAN PARENTS AGAINST NYS TIER 4 THAT FORCES US TO CHANGE MSC/CARE MANAGERS
The Issue
"New york State is forcing Children /Teens/ Adults who have OPWDD Waivers for Self Direction change MSC/Care Managers (now known as CCO Life Plan care managers) and forcing MSC/ Care Manager to drop Tier 4 clients. and giving these clients to another care manager, even if theses MSC have been with their current clients for many years.
NYS Is telling MSC/Care Managers that if they have any tier 4 clients on their case load they have to drop all their other clients yet the new MSC/Care Manager can have up to 20 tier 4 on a case load. ("NYS says they can't do both") although NYS clearly said that this new transition into (Life Plan CCO NY) would not affect their current service or who provides these services to our Family Members. (See quote below) NYS also want the MSC/Care Managers to do this so they can add up to 40 clients who are only tier 1,2,3, on the already filled or over filled 20-30 case load that MSC/Care Managers already endure. Although some who are a Tier 4 level in Self Directed services are not problematic as a tier 1,2,3, could be, such as my own child with or without multiple medical conditions. yet The new MSC/Care Manager can have up to 20 Clients who are a Tier 4 which they would be required to have a monthly face to face with each client. None of this makes sense to me and I assume other advocates who are dealing with this Life Plan transition.
Is NYS using Medical Conditions to justify the removal of a current MSC /Care Managers from their clients care plan that are a tier 4.
NYS has claimed along with Life Plan CCO this transition is always about the client ,and for them to have the right to choose who their support and service provider are this seems to be a infringement on their right to choose.
Life pain CCO states over and over in the CCO Life Plan policies that these Individuals have a right to choose who their providers are and no change to service plans.
I'm assuming most and for myself and my child that we want to keep our current service care provider ( Msc/Care Manager) no matter what tier they are. They should not be forced to change and be categorize by NYS based on their medical conditions and disabilities and should be able to choose freely as they have done in the past. If a client lower's to a tier 1, 2,3, this directly affect's their service plan and would also reduce the funding that a tier 4 clients would receive from the
(NYS Self Directed Services) (Services that are currently in place.) below is a statement from NYS web sight that there would be no changes in service plans or providers through this transition into Life Plan CCO NY.
NYS is forcing MSC/Care Managers to drop Clients this to me brings about a change. If at tier 4 can be reduced to a tier 3 not only recoups money for the state but also brings about change to our family members care plan. People would get to stay with their current MSC/Care Manager.( I could be wrong but it seems to me that is why NYS is doing this is all money motivated and forcing our hands to choose, what else makes sense.
Personally I believe my child's current MSC/Care Managers don't do any more for or less for my child than she does for any of her tier 1,2,3,clients probably less for her.
So why is the state making my child's MSC drop her only tier 4 client? Is it possibly (Money) motivated so that they can add more clients on their already hectic case load instead of hiring more employees to balance the change. or to get clients to drop to lower tier to save money?
In the end this does not look like Life Plan CCO is protecting the clients rights to choose and their rights to their services and providers with out change as they claim.
I am Begging you to please sign and forward this Petition and help me and other family's advocate for our loved ones to keep the same MSC/Care Managers that they currently have and let this be the clients choice and help protect their right to choose providers isn't this what the whole Life Plan CCO is all about clients right to choose/ Self Direct.

"If you are currently receiving MSC or Plan of Care Support Services (PCSS), you will need to sign a form to enroll in the CCO of your choice by July 1. Your current service coordinator will help you with this process. You may choose the CCO that is working with your current MSC, or you can choose a different CCO that serves people who live in your county. There will be no changes to the current authorized services you receive for your developmental disability, or who can provide them, as this transition from MSC to Health Home Care Management takes place."
