In order to get a "Grilled Cheese Sandwich" at McDonald's, you have to order a cheeseburger and then ask them to hold the meat.  Invariably, the cashier has a difficult time relaying the order and the kitchen has a difficult time making the order as requested. 

This is a problem for at least a couple of reasons.  First, the customer is forced to pay for meat she never wanted.  Second, the required litany of notations on the order (cheeseburger, hold meat, pickle, catsup, mustard) results in less than a 30% success rate.  As a result, this forces the customer to double check the order, thereby slowing down the line.

The solution is straightforward. In the interest of responsible stewardship and excellence in customer service, McDonald's ought to add "Grilled Cheese" to the menu.
