Justice for Marcus Malachi - Call for an Inquest into his Death

Our brother Marcus was a very intelligent, loving and gentle man. He suffered a mental breakdown in 2017. Marcus tragically committed suicide on 10 May 2020. He was 27 years old. Many questions still remain unanswered:

  • Marcus had been in and out of psychiatric hospitals for over 2 years and had a history of suicide attempts. He attempted to take his life 3 times between 29-30 April, a few days before his eventual suicide.
  • On the 2 May, Marcus was placed under Section 2 order (prevention of liberty) at Maudsley Psychiatric Hospital.
  • On 6 May, Marcus was discharged from Maudsley Psychiatric Hospital and doctors lifted the Section 2 under the Mental Health Act. This decision was completely against the wishes of Marcus’ family. Doctors argued that Marcus had ‘an improved mental state’. Why did professionals at Maudsley Psychiatric Hospital in Southwark refuse his family’s request to keep Marcus under a Section 2? (prevention of liberty order). Within 24 hours Marcus admitted himself as a voluntary patient to Maudsley Psychiatric Hospital.
  • On 8 May, Marcus was transferred from Maudley Psychiatric Hospital to Lambeth Hospital. Marcus’ aunt called and requested for Lambeth Hospital staff to call a family member if Marcus was to leave the hospital. On 10 May, a family member called and was told that Marcus left Lambeth Hospital around 1pm and was expected back within the hour. Our family were not notified. By 4.30pm Marcus had committed suicide. Why didn't hospital staff notify Marcus’ family that he had left the hospital? What mental capacity assesment, if any, was conducted before allowing him to leave???

Why Sign this Petition?

Marcus’ family and friends are devastated by the multiple failings made by the professionals involved in his care. We need to know the details of what went wrong. We are therefore asking that Lambeth and Southwark Council conduct a full inquest into Marcus Malachi's death. We hope that an inquest will push Lambeth and Southwark Councils to URGENTLY review their safeguarding policies and practices concerning the care of their most vulnerable mental health patients. We do not want Marcus' death to count for nothing. We want to prevent further unnecessary suicides. Please support us by signing this Change.org petition
