We, the undersigned community members and patients of David W. Malka, M.D., P.A., dba The MIND-Malka Institute of Neuroscience and Disease (hereafter referred to as "The MIND"), are writing to express our strong support for the continued inclusion of this practice as an in-network provider with Florida Blue.

For 30+ years, The MIND has provided compassionate, high-quality neurological care to patients across our community. The providers and staff have earned our trust through their professionalism, accessibility, and personalized attention to our health needs. Your unilateral decision to discontinue this vital partnership would:

• Disrupt continuity of care for countless patients,
• Force many to seek alternative providers in already overburdened networks,
• Increase out-of-pocket costs for services we’ve come to depend on,
• Undermine the long-standing doctor-patient relationships that support our health and well-being, and
• Negatively impact the health of numerous FloridaBlue and other Blue Cross Blue Shield network members. 
   

We respectfully urge Florida Blue to immediately reconsider any decision(s) that removes The MIND from its network. Maintaining this partnership is not only in the best interest of your members — it is essential to ensuring our open access to trusted, patient-centered care.

Please prioritize the health of your members by keeping The MIND as a contracted provider.
