Documenting Experiences, Concerns, and Outcomes

This survey is intended to document the experiences of current and former participants, family members, loved ones, former staff, and others with firsthand or secondhand knowledge of John Volken Academy (JVA).

The goal is to preserve individual accounts and identify patterns or recurring concerns across different JVA locations, time periods, and circumstances. Questions cover areas such as treatment and discipline, mental health and substance use, medical care, family involvement, work and labor, religious or personal beliefs, perceived differences in treatment, and experiences before, during, and after participation.

You may answer as much or as little as you feel comfortable sharing. If a question does not apply, you may answer “N/A.” If you are answering based on someone you know, please indicate when you are uncertain or relying on information they shared with you.

Responses are intended to remain confidential and for internal review unless you specifically give permission for selected information to be shared for legal, advocacy, educational, or public-awareness purposes.
