私たちは厚生労働省や製薬メーカーに対して、HIV感染を薬で予防する方法、PrEP(プレップ)を必要とする人が確実にアクセスできる環境整備のため、以下の3点を要望します。

 

1. PrEPへの安心・安全なアクセスを実現してください！
2. 経済的に厳しくても入手できる価格にしてください！
3. 医療従事者、支援者を含め、すべての人が正しいPrEPの知識を得られる啓発・情報発信をしてください

 

HIVは、薬で予防できることをご存知ですか？

HIV感染を薬で予防する方法PrEP（プレップ）は、これまで海外で幅広く行われてきました。HIVに感染していない人がPrEPの薬を服用し医療機関で定期的な検査を受けることで、HIVに感染するリスクを大きく減らすことができます。世界では、コンドームの使用に加えて、PrEPもHIV感染予防の強力な選択肢となっています。

PrEPの薬は、2012年にアメリカFDA(アメリカ食品医薬品局)で承認され、2015年にはWHO(世界保健機関)による推奨もはじまり、2021年までに144カ国がPrEPを国内の対策に採用し、世界中で広く使われてきました。また各国で行われた医学研究からも、HIV感染予防の効果は科学的に示されています。

 

8/28ついに日本でもPrEPが薬事承認。しかし課題も残る。

そうした中、ついに2024年8月28日、日本でもHIV感染を薬で予防する方法のPrEP(PrEPとしてのツルバダ)が薬事承認されることが決まりました！

しかし、薬事承認されても、まだPrEPの薬は予防目的の使用のため、制度上、健康保険の適用外になり、高額になることが予想されます。

PrEP in Japanの調査によれば、回答者の7割の人がPrEPでのHIV感染予防を希望していることがわかりました。そして「PrEPの薬を服用するとしたら、気になることは次のうちどれですか？」という質問に90％を超える人たちが「どれくらいお金が必要か」と回答しました。そして半分以上の人が、「PrEPに対して 1か月最大いくらまでなら払いますか？」という質問に、5000円未満と回答しています。

つまり、先述の調査で約7割の人がPrEPを使用してHIVを予防したいと思っているにも関わらず、高額で使えない人が多く出てしまう可能性がある、ということです。

せっかく薬事承認されるのに、薬の値段が高すぎて使えない人が大勢出てしまうと思われます。せっかくの薬事承認が逆の効果をもってしまうことは避けなければなりません。

またこのことは必ずしもPrEPを積極的に使う意志のある人だけの問題ではありません。HIVが社会的影響力の大きな感染症であるため、PrEPの使用を考えている人々だけではなく、その性的パートナー、HIV陽性者や家族など周囲の人たち、医療や行政などたくさんの人に関わる社会的な課題です。

PrEPによる恩恵を受ける人がこの日本にもたくさんいます

私たちはHIVの感染予防や、陽性者支援、性の健康を推進する活動を行っている団体の集まりです。

これまで日本でも研究のレベルでPrEPが行われてきました。PrEPを利用している人たちからは次のような声が寄せられていました。

• 相手に頼らず予防したかったから、PrEPのおかげで安心できるようになりました
• PrEPを始めてから、自分の心と身体の健康により意識が向くようになりました
• HIV・性感染症の検査結果を聞くのが毎度怖くて本当に嫌だったけれど、今では定期的に検査を受ける習慣がつきました

PrEPによる恩恵を受ける人がこの日本にもたくさんいます。費用を理由に、「HIVを予防したい」、その気持ちをあきらめなくてすむように、そしてPrEPがもっと広く知られ、使いやすくなるように。

 

私たちは厚生労働省や製薬メーカーに対して、PrEPを必要とする人が確実にアクセスできる環境整備のため、以下の3点を要望します。

 

1）PrEPへの安心・安全なアクセスを実現してください！

PrEPを必要とする人は、日本国内にたくさんいます。一人でも多くの人に届けるために、PrEPに使用する薬の供給を安定させることを求めます。行政と製薬企業それぞれが、希望する人々が安全にPrEPにアクセスできる体制を整えてください。またPrEPとしてのツルバダの薬事承認により、ツルバダのジェネリック薬の国内での流通がなくなる見通しです。しかし価格の面から、国内で海外製のジェネリック薬が選べることも必要です。

２）経済的に厳しくても入手できる価格にしてください！ 

月に5,000円を負担するのが難しい人でも、PrEPを安価に利用できる制度を行政と製薬企業が協力して構築してください。経済的な理由でPrEPという予防方法をあきらめることがないようにしてください。

３） 医療従事者、支援者を含め、すべての人が正しいPrEPの知識を得られる啓発・情報発信をしてください

医療従事者、支援者を含む、すべての人に向けて、行政と製薬企業がそれぞれPrEPに関する情報の啓発を促進してください。医療機関等でのPrEPを必要とする人への処方や検査と見守りの機会を増やし、使用者への差別・偏見がないようにしてください。

手記①

　少ない手取りの中からPrEP薬を個人輸入することや、性感染症の検査にかかる費用を個人負担するのは、大きな痛手です。PrEPが経済的に豊かな人たちにだけアクセス可能な予防手段とならないように、若い世代や障害のある人たちにも確実に使える、選ぶことのできる予防手段のひとつとして、日本で導入されることを強く願っています。

手記②

　PrEP はウェブサイトから個人輸入して服用していますが、物流の状況に左右されて手元に届くまでにかなりタイムラグが発生する場合があり、国内で確実に、かつ安価に入手できる方法があれば良いなと感じています。海外にときどき行くのですが、パリで出会ったフランス人は「パリでは検査もPrEPの薬も無料だよ、その方がトータルで医療費も抑えられるし、健康が守られる人が増えるからね」と言っていました。日本でも、より多くの人が安全に使用できる体制が整うことを望んでいます。

　私は、双極性障害と診断されていて、躁状態のときに性行動がアクティブになる傾向があります。PrEPを始める前は、住んでいる街の保健所でHIV検査を受けていましたが、結果を聞きに行くことが怖くて本当に嫌でした。「陽性です」と告げられるのではと強く思い込んで、結果を聞きに行かなかったこともありました。

Did you know that HIV can be prevented with medication?

 

PrEP (Pre-Exposure Prophylaxis) is a method of preventing HIV through medication, which has been widely used in other countries. Taking PrEP medication along with regular medical check-ups can significantly reduce the risk of contracting HIV in uninfected individuals. Used in conjunction with condoms, Around the world, PrEP has become a powerful option for HIV those not prevention, alongside the use of condoms. globally.

 APrEP was approved by the U.S. FDA in 2012 and , recommended byy the WHO as a highly effective HIV prevention choice sincein 2015, and by 2021 PrEP had been adopted by, 144  countries worldwide had adopted PrEP as part of their national HIV prevention strategies.

 

In 2024, Japan will finally approve PrEP for HIV prevention!

However, because it is used for prevention rather than treatment, PrEP will not be covered by health insurance, even after approval. This means it will likely be expensive.

 

According to a survey regarding PrEP in Japan, 70% of respondents want to use PrEP for HIV prevention. Yet, when asked about their concerns regarding taking PrEP, over 90% asked, “How much will it cost?” More than half of them stated they could only afford to pay less than 5,000 yen per month for PrEP.

 

This means that although many people want to use PrEP to prevent HIV, its high cost could prevent them from accessing it. Even with approval, the high price could make the medication unaffordable for it.

 

We, the undersigned leaders of this campaign, are a group of organizations dedicated to HIV prevention and support for people living with HIV in Japan. Until now, PrEP in Japan has been limited to research use, with participants sharing the following personal experiences:

 

•  ・"I wanted to protect myself and not rely on a sexual partner. Thanks to PrEP, I can now feel at ease."

 

•  “Since starting PrEP, I’ve become more conscious of my mental and physical health.”

 

•  “I used to dread hearing the results of HIV/STI tests, but now I get tested    regularly without fear.”

We don’t want people them to be forced to have to give up onforgo HIV prevention ng HIV due to financial constraints. PrEP should needs to be more widely known and accessible.

 

We call on the Ministry of Health, Labour and Welfare and pharmaceutical companies to ensure that those who need PrEP can obtain it by addressing the following three pointsissues:

 

1.  Ensure Safe and Accessible PrEP!

Many people in Japan need PrEP. We urge you to stabilize the supply of PrEP medications to ensure that as many people as possible can access itthem. We ask for cooperation between the government and pharmaceutical companies to establish a system that enables people to obtain PrEP safely. Until a stable supply chain is established, we request flexibility in allowing the distribution of overseas generic PrEP medications within Japan.

 

2.  Make PrEP Affordable, Even for Those with Financial Difficulties!

We ask the government and pharmaceutical companies to work together to create a system where even those who find it difficult to pay 5,000 yen per month can access PrEP at a lower cost. No one should have to give up this vital prevention method due to financial reasonsconcerns.

 

3.  Promote PrEP Education and Information Sharing, about PrEPMaking It Accessible to All, Including Healthcare   Professionals and Support Workers!

We urge you to promote PrEP awareness and education across all communities and social sectors, including healthcare professionals and support workers. Additionally, we request broader access to PrEP prescriptions and monitoring at medical institutions, ensuring that those in need can receive PrEP services without discrimination or stigma.

1.           Ensure safe and secure access to PrEP!

Many people in Japan need PrEP. To ensure PrEP reaches as many people as possible, we call for a stable supply of the component drugs used in PrEP. We urge the government and pharmaceutical companies to collaborate on establishing a system that enables safe use of PrEP. While Truvada’s approval as a PrEP medication is expected to end the domestic distribution of generic versions, it is necessary to continue allowing access to these overseas generic drugs in Japan due to cost considerations.

 

2.           Make PrEP affordable for all, including those facing financial difficulties!

We call on the government and pharmaceutical companies to work together to create a system whereby even those who find it difficult to pay 5,000 yen per month can access PrEP at a lower cost. No one should have to give up this vital prevention method due to financial concerns.

 

3.           Promote education and information dissemination so that everyone, including healthcare professionals and support workers, can obtain accurate information about PrEP!

We urge the government and pharmaceutical companies to promote PrEP awareness and education across all communities and social sectors, including healthcare professionals and support workers. Additionally, we request broader access to PrEP prescriptions, testing, and monitoring at medical institutions, ensuring that those in need can receive PrEP services without discrimination or prejudice.
