  • My father Peter Yu died in NORTH YORK GENERAL HOSPITAL of Toronto Canada on Sep 28 due to **unhandled cardiac tamponade** for over 72 hours.
  • No telemetry. No oxygen.
  • He died in ward with no nurse attended. The nurse even suspectedly forged the notes.
  • The doctors and the hopstials have no answers and they’re still rightfully harming people everyday!!
  • No one told my mom he was that severe until we learnt from the medical records. My mom collapsed on the road to the hospital in a morning when she heard my father passed. Up to date no mainstream media has covered. 
  • We're still being silenced by the policitians and the mainstream media.

We, the undersigned—grieving families, patient safety advocates, compassionate citizens, and everyone who believes no one should be left alone in their final hours—stand with Kenny (me below, as the petition starter) to demand truth, accountability, and real change after the tragic and preventable death of my father, Peter Yu, on September 28, 2025, at North York General Hospital in Toronto, Ontario.

My dad was a devoted husband to my mother for decades, a loving father to me and my sister, and a cherished grandfather to my niece and nephew. He was kind, hardworking, gentle, and always put family first—his quiet strength and warmth touched everyone around him. On that heartbreaking day, he passed from untreated cardiac tamponade that went unaddressed for over 72 hours. Despite clear signs of distress, he received no telemetry monitoring, no supplemental oxygen, and was left unattended and unmonitored for extended periods—including over 6 hours without any nursing checks, as shown in the medical records. He was found cold and unresponsive the next morning, having passed alone with no one there to attend to him or offer comfort. The severity of his condition was never communicated to us—we only learned these painful details afterward. My mother collapsed on the road to the hospital upon hearing the news, and that grief continues to weigh heavily on our family.

In the months since, my own efforts to seek answers have revealed what feels like deep barriers that shield institutions rather than support patients and families:

  1. Persistent silence and lack of meaningful explanation from the hospital.
  2. No coverage from mainstream English-language media, despite outreach and existing reporting by
  3. Political inaction and cover-up
    • riding MPP Stan Cho literally said "I can't help" on phone
    • also riding MPP Stan Cho quietly cancelled a healthcare town hall in December 2025 where CEO of the hospital would attend, claiming it a "technical difficulty".
  4. Apparent suppression of open discussion (e.g., Reddit threads removed despite strong community engagement like 1.2 upvotes and hundreds of replies).
  5. Significant obstacles to legal accountability, often linked to the resources and protections provided to physicians through systems like the CMPA.

These patterns—of inaction, silence, and what appears to be cover-up—deny grieving families closure and allow preventable tragedies to repeat. My dad's story must become the turning point for protection and reform.

I respectfully demand:

  1. An immediate, fully independent external investigation into my father's death (conducted by a body unaffiliated with North York General Hospital or the CMPA), with complete public disclosure of findings, care timelines, monitoring failures, and any lapses in attendance or communication.
  2. Clear accountability if negligence, inadequate oversight, poor staffing, or misconduct contributed—including appropriate disciplinary actions where warranted.
  3. Urgent systemic reforms to protect every patient: mandatory family notification of critical changes, required telemetry and oxygen monitoring for high-risk cardiac cases, consistent nursing attendance and check protocols, stronger oversight of care decisions, and safeguards against the silencing of legitimate patient safety concerns.
  4. Better support for families in crisis: transparent and timely access to records, compassionate communication from the start, and fair pathways to answers and justice when trust is broken.

My dad's life mattered deeply. His memory deserves truth, honor, and the assurance that no other family will face this same silence and pain. By signing, you help amplify my voice, raise awareness from my real experience, and help build a healthcare system that truly attends to those in need.
