Thank you for your inquiry regarding a Utah Special Group Plate.

The plate application and instructions will be available at https://dmv.utah.gov/plates/delete-utah-special-group-plate/.

This website is currently under construction. We are changing the website to add the new application, fees and instructions. This should be available soon. Hopefully within the the next month.

Here is the process so that you can be ready when the website is finished.
Submit an application with the information required by statute. Make payment of $16,000 ($3,250 for administrative costs, $7,750 for implementation and system programming costs, and $5,000 for design implementation). After the application is reviewed and approved, the Special Group Plate is added to our Motor Vehicle Portal. In order for a plate to be manufactured, 250 people need to show interest in purchasing the plate.

In the Motor Vehicle Portal, people can sign up to show interest in the plate. Once 250 people have signed up, you can submit a plate design using the standard plate design template which will be available on the same website listed above. If you don't have a design, you can work with our designer to come up with a plate design.

The design will then need to be reviewed and approved by the Design Review Board. Once the board approves the plate design, the Division will have up to 6 months to make the plate available for the public to order. Once the plate is available to order, our system will email the people who showed interest to let them know they can go order the plate.

The division shall disburse the collected contributions to each sponsoring organization at least annually.

(1) completed W-9
(2) a copy of the IRS letter approving your Section 501(c)(3) status
(3) an affidavit, signed under penalty of perjury, declaring that you have a charitable purpose
(4) payment of $16,000 ($3,250 for administrative costs and $7,750 for implementation and system programming costs, $5,000 for design implementation) - make check out to Utah State Tax Commission
