Withdraw Fabricated Induced Illness (FII) Guidelines: Protect Families From Harm & Abuse
The Issue
To: The Secretary of State for Health and Social Care, UK Government
Date: October 5, 2025
We, the undersigned concerned citizens, parents, healthcare professionals, and advocates for family rights, call upon you to urgently withdraw the current guidelines and practices surrounding Fabricated or Induced Illness (FII) allegations. These reforms are essential to prevent the irreversible harm caused by unfounded accusations that tear families apart, erode trust in child protection systems, and inflict lifelong trauma on innocent children and parents.
The Urgent Issue: A Wave of False Accusations Destroying Lives
Fabricated or Induced Illness (FII), previously known as Munchausen syndrome by proxy, refers to rare cases where a caregiver fabricates or induces illness in a child to gain attention or sympathy. While genuine instances must be addressed to safeguard vulnerable children, the current diagnostic and procedural frameworks have led to a alarming surge in wrongful allegations against loving parents and carers. Families report being subjected to invasive investigations, sudden child removals without due process, and stigmatization that shatters their lives—all based on subjective interpretations of medical symptoms, particularly in children with complex or undiagnosed conditions like disabilities or rare diseases.
Research from Cerebra's 2023 report reveals that the RCPCH's 2021 FII guidance is prone to generating a "very high number of 'false positives,'" disproportionately affecting parents of children with genuine but perplexing medical needs. These errors have resulted in "devastating and life-long impact" on families, including mental health crises, financial ruin, and the permanent severance of parent-child bonds. One family's story exemplifies this tragedy: a mother's desperate fight to prove her child's real illnesses led to years of separation and emotional devastation, as false claims "ripped the family apart."
Experts, including those contributing to a 2025 critical report, warn that outdated and overly broad criteria enable professionals to "fabricate the truth" through biased assessments, exacerbating the problem rather than resolving it. Campaigners describe this as an "extreme form of parent blame," particularly targeting mothers who advocate fiercely for their children's care. A recent investigative deep dive highlights how such inaccuracies have affected numerous families, calling for systemic change to restore fairness.
While we acknowledge the critical need to protect children from true abuse, the imbalance in the current system prioritizes suspicion over evidence, leading to miscarriages of justice that undermine public confidence in healthcare and social services. In May 2025, leading experts urged the development of new official guidelines to replace those proven "inaccurate and the cause of false accusations against parents."
Our Demands for Immediate Action
To prevent further injustice and ensure child protection is both compassionate and evidence-based, we demand the following reforms:
Withdraw FII Guidelines Immediately: Update RCPCH and Department of Health protocols to incorporate rigorous, multi-disciplinary evidence thresholds, mandatory second opinions, and explicit safeguards against false positives—particularly for families with disabled or chronically ill children.
Mandate Independent Reviews and Due Process: Require all FII allegations to undergo prompt, impartial external review before any family separation, with full transparency and legal representation for parents.
Enhance Training and Accountability: Provide mandatory, bias-aware training for healthcare and social care professionals on recognizing genuine complex conditions versus rare fabrication, and establish clear mechanisms for investigating professional misconduct in wrongful allegations.
Support for Affected Families: Launch a national compensation and counseling fund for families exonerated of FII claims, and conduct a public inquiry into the scale and impact of false allegations to inform future policy.
Public Consultation: Initiate a broad stakeholder consultation, including parents, medical experts, and disability advocates, to co-design "a fair way forward" in handling FII cases.
These changes will not only protect innocent families but also strengthen the system's ability to identify and address true cases of harm, ensuring no child falls through the cracks while no family is unjustly destroyed.
We urge you to act with the urgency this crisis demands. Families cannot wait—every day without change
means more lives shattered.
