"Specialist nurses play a vital and far-reaching role. Their extensive knowledge and expertise allow them to work across multiple specialist areas, assessing, referring, and collaborating with patients and colleagues.

Beyond their specialty, they also have a powerful influence in championing the highest standards of fundamental care. Despite the evolving nature of their roles, often encompassing many responsibilities, their job descriptions and pay do not reflect these changes.

The grading system places a ceiling on their advancement, and many specialist nurses, driven by their passion for patient care, have no desire to move into management roles.

There must be a clearer career pathway for specialist nurses, with better succession planning in place, especially as many are nearing retirement."


I recently achieved a significant personal milestone when speaking at a European conference to an audience primarily made up of medical professionals. At the same event, I presented a poster showcasing the work we are doing. I was humbled to receive recognition and praise for advocating for specialist nursing and raising our profile as experts in the field. However, despite this success, my salary has remained stagnant for 14 years, as I reached the ceiling of Band 8a and had no opportunity to apply for an 8b nurse consultant role in my workplace.  

Disturbingly, as specialist nurse colleagues have left, they are being replaced by specialist practitioners, many of whom are not nurses, effectively altering the role of specialist nursing. This shift has been enabled by the lack of a formal structure for specialist nursing in the UK. As a result, specialist nursing posts have been terminated, downgraded, diluted, or replaced, leading to the loss of invaluable expertise within the NHS.  

I am calling for a comprehensive review of specialist nursing roles and urging that specialist nurses from across the country be included in this process. Scotland has already begun the process of transforming these roles, and it is essential that we work with colleagues to implement similar changes in England, Wales, and Northern Ireland. The future of specialist nursing depends on it.
