The Issue

Our current National Insurance system treats everyone the same, regardless of their personal health risks or lifestyle choices. This one-size-fits-all approach is outdated and unfair.

Across the UK, many people take proactive steps to stay healthy — by exercising regularly, eating a balanced diet, avoiding smoking, limiting alcohol, and attending health check-ups. These actions greatly reduce the risk of chronic illness and significantly lower their demand on the NHS.

Yet, despite making responsible lifestyle choices, these individuals still pay the same National Insurance contributions as those with far higher long-term health risks.

In other areas — like car insurance — safer drivers are rewarded with lower premiums. It’s time our National Insurance system applied similar logic: those who reduce risk should be rewarded.

We’re calling on the government to explore a fairer, incentive-based model for National Insurance — one that offers reduced contributions or benefits for individuals who actively maintain a healthy lifestyle and place less strain on the NHS.

This could be achieved through secure, voluntary tracking methods such as:

  • Opt-in integration with digital health apps and wearables (e.g., step count, activity levels)
  • Participation in NHS or local health programmes (e.g., smoking cessation, Couch to 5K)
  • Preventive care compliance (e.g., vaccinations, screenings, medication adherence)
  • Annual self-declarations via the NHS App, reviewed by healthcare professionals
  • A future points-based “Wellness Passport” system to track healthy habits

Key Benefits of This Proposal:
✅ Reduces pressure on the NHS by lowering preventable illness rates
✅ Cuts NHS wait times by reducing demand for appointments and hospital care
✅ Saves billions in long-term public health costs
✅ Promotes prevention over cure
✅ Rewards personal responsibility
✅ Supports a healthier, more productive nation — healthier people can contribute more to the economy through better work attendance and higher productivity

This isn’t about punishing anyone — it’s about creating incentives for healthy choices, reducing preventable disease, and supporting a sustainable NHS.

Let’s make our National Insurance system smarter, fairer, and more aligned with how people actually live.
