This certificate was created out of desperation for this employee to return to work without proper training to fulfill staffing crisis in Dec 2025. Although Caldwell claimed to be unaware of this certificate in her interview with the state, she was not being honest. She indeed asked for this certificate to be created and signed by the nurse for this employee due to the need for staffing in the building. She also has forged other documentation or had it forged but had the tenacity of the audacity to report staff to county and state on a 24/5 claiming fraud against the facility. The only fraud is Caldwell and all her accomplices.
