The Issue
On June 6, 2026, my world changed forever when my husband, Mr. Shiv Shankar, a senior citizen, was admitted to the Institute of Liver and Biliary Sciences in Vasant Kunj, Delhi. He was suffering from fever on that day, a condition we believed would soon stabilize with appropriate medical care. However, on June 13, just a week into his hospitalization, he tragically passed away.
We were devastated by his unexpected departure, but our grief turned into a deeper resentment when we discovered a horrifying detail posthumously. An infusion syringe marked with another patient’s sticker was attached to his body. This was a blatant act of gross medical negligence, one that should never befall a patient. That was not the only one but other acts of negligence and misdiagnosis such as terming common cold as COVID-19, administration of insulin to a non-diabetic patient, Breach of EMR on next day of death, discrepancies in physical records, were also committed by the hospital staff. 2+ months passed, authorities still sitting on the Complaint against this powerful and influential institution.
Hospitals are sanctuaries for healing. They are places we trust with our lives and the lives of our loved ones. Medical negligence not only breaches this trust but also endangers lives. It is crucial that healthcare institutions are held accountable to prevent such fatal mistakes from reoccurring.
The Institute of Liver and Biliary Sciences must be investigated thoroughly for this grievous error. Medical staff involved should be subject to rigorous questioning, and corrective measures must be promptly implemented. We demand full transparency and accountability from the institution. The safety protocols must be revised and strengthened to ensure that no other family faces such an irreversible loss.
Moreover, there should be stringent regulations in place to monitor and prevent drug administration errors across hospitals in India. Statistics from the World Health Organization reveal that unsafe medication practices and errors are a leading cause of injury, accounting for substantial preventable harm to patients worldwide. Raising awareness and ensuring strict compliance to best practices can drastically mitigate these risks.
It is imperative that the Delhi Govt, DMC, National Medical Commission, NHRC and Delhi Police acknowledge the seriousness of this incident. To honor my husband and to safeguard countless others, we must act now. A detailed investigation must commence immediately, followed by appropriate actions against those responsible.
