There is a lack of data and accurate numbers for adverse drug reactions in Canada. Adverse drug reactions are reported to the Canada Vigilance program on a voluntary basis by health professionals and consumers. Don't assume that your health professional has reported it .You need to report it yourself or it may be overlooked. 
Coroner’s for death by suicide only test for a set amount of drugs. If you don’t know your loved one was taking a pharmaceutical drug, you won’t know to ask for a post mortem test. It may have caused/contributed to your loved ones death but this will be overlooked and the data never collected. 
I used the estate lawyers letter to receive my daughters medical records. 

Suspected adverse reactions to drugs marketed in Canada, including prescription and non-prescription drugs can be reported to the Canada Vigilance Program. Follow the instructions for reporting adverse reactions provided in the MedEffect Canada section of the Health Canada Web site.
